Every Failed Check Keeps Its Ticket

A run of fixes to the moment a failed check becomes a ticket. Cancelling the detail dialog no longer throws away the retry, two people working the same audit can no longer write a ticket over each other's, and the walkthrough note now describes what was actually recorded rather than what was left in the cache.
